2000 Grand Marquis lurching when shifting from 3rd to 4th gear by hjf05a

When my OD is on my CW lurches heavily and won't/has trouble shifting into 4th when I hit around 35mph. I have to manually work the gas to get it to upshift. I'm also getting a squeaking/creaking noise while driving.
 
This all goes away when I disable my OD. I already checked my transmission fluid and it's fine.

Change the transmission fluid with Mercon V, and make sure they drain the torque converter as well - the 2000 still has a drain plug. I have my shop drop the pan and change the transmission filter every 60,000 miles as well.
 
Mercon V fluid is backward compatible, and your symptons suggest old fluid could be the cause.
