How do I release the rear pads (The self adjuster)?

Only one plug on the back of the drum. Its at the edge and when removed you see the edge of the shoe (not an adjuster). It appear to be a way of viewing the condition of the shoes. There doesn't appear to be an adjuster.
Further, I took of the wheel and the center nut from the opposite side. That bearing looks fine and I can't get that drum off either.
I'm stumped.

Here's how you remove the drum...straight from the service manual...
 
1.Release parking brake.
2. Apply the brake pedal at least 10 times.
3. Raise and support vehicle with jack stands, etc.
4. Remove the tire and wheel assy.
5. Remove the lock ring and the caulking nut from the spindle.
6. Remove the drum from the spindle.
Note: Pull the drum straight off thge spindle. The 2 inner wheel bearing races are not secured inside the drum by the ring.
7. Remove the wheel bearing, if necessary.
