Well you'd need a truck with a ladder frame, that's for sure, not a unibody. Then I guess it's a matter of engineering a cradle for the engine in center-frame, and of course a short driveshaft, some kind of engine cover, throttle linkage, shift linkage. It would be a challenge to one's fabrication skills!
Seems to me that any off-roader would need a long travel suspension kit and some way to widen the tire stance.
